  • To obtain a copy of the Collision Report filed by a Police Officer  within the city limits, contact the local police department in the city the auto accident happened in. 
  • To obtain a copy of the Collision Report filed by a Deputy Sheriff outside of the city limits, contact the sheriff's office in the county the auto accident happened in.​
  • To obtain a copy of the Collision Report filed by an Oklahoma State Trooper on an interstate, contact the Oklahoma Department of Public Safety.  Here is a link for the DPS form you can fill out and download on-line through the DPS website https://www.ok.gov/dps/documents/records_request.pdf.  You will need to complete this form for DPS to get the report mailed to you or to get it in person.
